The temporary reimbursement of Kalydeco has been prolonged. As a result, registration at extra follow-up moments is needed.
To this purpose, a version 2.0 of the registration form has been created.
The existing (v1.0) registrations need to be manually converted to v2.0 registrations by following the below steps.
Additional follow-up moments are added to each registration:

All existing registrations in version 1.0 should be
converted to version 2.0.
The following instructions will lead you through the process of the conversion:
- Step 1: download the registrations in version 1.0
- Step 2: upload the CSV for activating the additional follow-up moments in version 2.0
- Step 3: submit uploaded registrations in version 2.0
- Step 4: completing the additional data in version 2.0
- Step 5: submit additional data in version 2.0
Step 1: download the registrations in version 1.0
Download all registrations in version 1.0 to a CSV file to capture the data:

A CSV file will be created. You will need this file in the next steps of the process.
Step 2: upload the CSV for activating the additional follow-up moments in version 2.0
The CSV file with the registrations needs to be uploaded into version 2.0. Go to version 2.0 and select
'CSV upload'.

When uploading a pop-up appears with
4 options:
- In case of conflict, take the data of the new registration
- In case of conflict, keep the data of the existing registration
- Keep the existing registration without changes
- Keep the new registration and delete the old record
Select the
first option and confirm by clicking the OK-button

The
result of upload is shown in the next pop-up:

The new records will be directly visible in the overview with status
'Open'. The registrations will
not have the follow-up mechanism activated.This means that only the data for the General and baseline data
fields are active and therefore is filled in using te CSV file the first time.
Step 3: submit uploaded registrations in version 2.0
The registrations which were uploaded from version 1.0 need to be
re-submitted for version 2.0 to
activate the follow-up mechanism.
Therefore select the registrations and submit them by using the Actions-button and 'Send registrations'. The option will become available when records are selected.

The new records will be directly visible in the overview with status
'In transit'. 
The sending process has been started and could take one hour before reaching status
'Follow-up needed'.
When opening a registration in this status, the follow-up milestones are
visible in the left pane and additional fields that relate to these milestones will become active.
Step 4: completing the additional data in version 2.0
The CSV file with the registrations needs to be uploaded again in version 2.0 to add the data of the follow-up milestones that have active fields related to the milestones.
Since the fields for these milestones did only become available when the registrations were correctly submitted is the previous step, we will need to upload the data again.
The data present in the CSV file that could not be loaded into the application the first time will be added automatically in the application now that the additional fields are active.
Go to version 2.0 and select
'CSV upload'.

When uploading the pop-up appears again with
4 options:
- In case of conflict, take the data of the new registration
- In case of conflict, keep the data of the existing registration
- Keep the existing registration without changes
- Keep the new registration and delete the old record
Select the
first option and confirm by clicking the OK-button

result of upload is shown in the next pop-up:
Step 5: submit additional data in version 2.0
The data that was added by the second upload needs to be
submitted again in order to update the databases with this data.
Only registrations that are
valid can be submitted. Registrations with errors need to be completed or corrected.
Add
column 'Errors' to the basic view in order to be able to select the registrations that are valid:

Select the registrations and submit them by using the Actions-button and 'Send registrations'. The option will become available when records are selected.

The new records will be directly visible in the overview with status
'In transit' and will change to
'Follow-up needed' or
'Submitted' depending on the '
Date of CF diagnoses' in the registration.
